Official Rules, Terms & Conditions

(“Sponsor,” “Moderne Medical,” “we,” “us”)

NO PURCHASE OR PAYMENT NECESSARY TO ENTER OR WIN. A purchase or payment

will not increase your chances of winning. Void where prohibited by law.

1. Eligibility. To be eligible, an applicant must, at the time of application: (a) be a current

high school senior or current college/university student; (b) plan to be enrolled full-time at

an accredited college or university in the State of Arizona for the 2027 academic year; (c) be

pursuing, or have declared intent to pursue, a course of study in healthcare or business; (d)

have and maintain a minimum cumulative GPA of 3.0 (on a 4.0 scale, or its equivalent); (e)

demonstrate a future goal oriented toward leadership and positive community impact; and

(f) be a legal resident of the United States. Employees of Moderne Medical, and their

immediate family members (spouse, parent, child, sibling, and their respective spouses) or

household members, regardless of where they reside, are not eligible to enter or win.

Sponsor reserves the right to verify eligibility and to disqualify any applicant who does not

meet these requirements at any stage of the process.

2. Application Period. The application period opens July 29, 2026 and closes September

29, 2026, at 11:59 PM Arizona Time (MST). Applications received after this deadline, for any

reason, will not be considered. Sponsor is not responsible for late, lost, incomplete, or

misdirected submissions.

3. Submission Requirements. This is a video-based application; no written essay is

required or accepted in place of the video. Each applicant must submit one (1) video, filmed

by the applicant, addressing the topics described above, to support@modernemedical.com.

Limit one (1) submission per applicant; multiple submissions from the same individual may

result in disqualification.

4. Selection Process. All eligible submissions will be reviewed by a selection committee

designated by Moderne Medical. Videos will be evaluated based on clarity of academic and

career goals within healthcare or business, demonstrated current and future leadership

involvement, sincerity and specificity of the applicant’s community impact plans, and overall

authenticity and communication. Selection decisions are made at the sole discretion of

Moderne Medical and are final. This is a merit-based scholarship judged on the criteria

above; it is not a game of chance.

5. Winner Notification & Award. The winner will be notified by phone and/or email using

the contact information provided in their application on or around October 2026. The winner

may be asked to provide proof of enrollment or acceptance for the 2027 academic year at

an eligible Arizona college or university, and/or proof of GPA (e.g., an unofficial transcript),

prior to disbursement. The $3,000 award will be issued directly to the winning applicant.

The recipient is solely responsible for any and all applicable federal, state, and local taxes

associated with the award. Moderne Medical will issue an IRS Form 1099 to the recipient as

required by law. If a selected winner cannot be reached, does not respond within a

reasonable time, does not meet eligibility requirements, or declines the award, Sponsor

reserves the right to select an alternate winner or to not award the scholarship for that

cycle.

6. Use of Video & Publicity. By submitting a video, applicants grant Moderne Medical a

non-exclusive, royalty-free right to use their name, likeness, city/state of residence, and

submitted video, in whole or in part, for promotional and marketing purposes related to the

Moderne Rise Scholarship, including on Moderne Medical’s website and social media

channels, without additional compensation, except where prohibited by law. Applicants may

decline this permission in writing without affecting their eligibility to apply; however, doing

so may limit how Sponsor is able to publicly announce the winner.

7. General Conditions. By applying, applicants agree to be bound by these Official Rules

and by the decisions of Moderne Medical, which are final in all matters relating to this

scholarship. Moderne Medical reserves the right to cancel, suspend, extend, or modify the

scholarship or these rules at any time, for any reason, including if it determines the program

cannot proceed as planned. This scholarship is offered only where legally permitted.

Moderne Medical is not responsible for technical errors, video/email delivery failures, or any

other issue affecting a submission’s receipt. These Official Rules are governed by the laws

of the State of Arizona.

8. Privacy. Information submitted as part of an application will be used solely for purposes

of administering the Moderne Rise Scholarship and will not be sold to third parties. Video

submissions may be used per Section 6 above.
